Built for Scottish general practice

More screening. Less GP appointment time.

Let patients complete a structured sexual health assessment online—so your team can move directly to the appropriate screening pathway without the need for a GP appointment.

The opportunity

There are many barriers to patients booking appointments for sexual health screening, and opportunistic screening can use a lot of GP appointment time. Screening Forms allows patients to access a thorough and structured assessment online with practical outcomes tailored to their level of risk. This removes barriers for patients and allows practices to optimise their screening capacity, bypassing the need for a GP appointment.

The traditional pathway

Start by gathering information

Clinicians are required to use appointment time to undertake routine screening—either as a pre-booked appointment for screening or opportunistic screening in an appointment for a separate issue. Time pressure can make it difficult to gather all the relevant information within the appointment time.

The people behind the idea

Built by the Gibsons.

We’re David and Mari Gibson. David works with Ocho, a creative design agency that builds websites and solves practical digital problems. Mari is a GP working in Inverness.

By bringing together clinical experience and digital design, we saw an opportunity to make routine sexual-health screening more efficient—for patients, reception teams and clinicians.

Privacy by design

Collect what is needed. Keep it only as long as needed.

The questionnaire is designed around UK GDPR principles of data minimisation, limited retention and privacy from the outset.

No patient names

The screening response is anonymous until the patient chooses to identify themselves directly to their practice. No patient-identifiable information is stored online.

Random identifiers

Each submission receives a unique random code and PIN, linking the response only when the patient makes contact.

Seven-day deletion

Questionnaire entries are configured for automatic deletion after seven days, reducing unnecessary retention.

Each participating practice remains responsible for completing its own information-governance review and confirming the final configuration and workflow.

A simple patient journey

From questionnaire to screening tests

01

Share the form

The patient has access to a link—sent by the practice or on the practice’s website.

02

Patient completes it

The patient answers the structured questions privately, without appointment pressure.

03

Receive an outcome

The agreed protocol provides an outcome, unique code and clear instructions for contacting the practice.

04

Use the visit well

The patient either collects the relevant swabs, or books a nurse appointment for bloods, or both.

Designed around practice needs

Efficiency without losing clinical oversight.

The tool supports the pathway; it does not replace clinical judgement. Protocols, patient guidance and local instructions are agreed with each practice.

Less repetitive questioning

Move routine information gathering outside the clinical appointment.

Greater screening capacity

Make a consistent screening pathway easier to offer at greater volume.

Optimise Enhanced Services

Support eligible locally commissioned activity where applicable.

Consistent decision protocol

Apply the same clinically agreed screening protocol to every submission.

Interactive demonstration
